The agency utilizes efficient staff scheduling, geographic assignment of caregivers, and coordination with other healthcare providers to reduce travel time and service overlap. Care plans are regularly reviewed and adjusted to reflect the client's current needs, ensuring resources are used appropriately.
Naviah Home Care Services also promotes client independence through supportive care strategies, reducing long-term reliance on services when appropriate.

Quality Assurance Mechanism and Activities

Naviah Home Care Services maintains a structured Quality Assurance (QA) program to monitor, evaluate, and improve service delivery. Quality assurance activities include:
- Routine supervisory visits and performance evaluations of caregivers

- Incident reporting and investigation procedures
- Corrective action plans for identified deficiencies

Caregiver Qualifications & Training

All caregivers complete an orientation that covers agency policies, client rights, infection control, safety, and reporting requirements. Training emphasizes respect for client autonomy and safety. Competency evaluations are conducted upon hire and on an ongoing basis to ensure continued proficiency.

Plan of Operation — Referral Through Follow-Up

Naviah Home Care Services follows a structured process to ensure seamless service delivery:

Referral & Intake
Referrals may come from healthcare providers, managed care organizations (MCOs), family members, or clients directly. Intake information is collected to determine eligibility and service needs.

Assessment
An initial assessment is conducted to evaluate the client's functional abilities, care needs, and preferences.

Care Plan Development
An individualized service plan is developed outlining the scope, frequency, and type of services to be provided.

Staff Assignment
Qualified caregivers are assigned based on client needs, location, and compatibility.

Service Delivery
Services are delivered in accordance with the care plan, focusing on safety, dignity, and independence.

Monitoring & Supervision
The Administrator or designated supervisor provides ongoing supervision through periodic in-home visits, communication with clients and caregivers, and review of service documentation.

Care plans are reviewed and updated as needed based on changes in client condition, needs, or preferences.

Documentation
The agency maintains documentation of all services including visit records, care plans, incident reports, and supervisory notes, in accordance with state requirements.

Statement of Services

Naviah Home Care Services provides non-medical personal care services to individuals in their homes, including but not limited to:
- Assistance with activities of daily living (ADLs): bathing, grooming, dressing, and toileting
- Mobility assistance and transfers
- Meal preparation and feeding assistance
- Medication reminders (non-clinical)
- Light housekeeping and household support
- Companionship and supervision
- Support for individuals with physical, cognitive, or developmental limitations
Naviah Home Care Services does not provide skilled nursing or medical services. All services are delivered in accordance with the individualized care plan and applicable Wisconsin regulations.
