Services
Naviah Care Services LLC delivers community-based support in the member's own home. Every task we perform follows the member's assessment, authorization, and individualized service plan — not a fixed package.
Service Line One
Day-to-day support, supervision, and household help that keep members safe and independent at home, provided according to authorization, service plan, and staffing.
Transportation and community accompaniment are not automatic — they are provided only when separately authorized and accepted by our agency.

Inside Supportive Home Care
These are not separate programs — they are the supports we combine within a member's supportive home care authorization.
Consistent, respectful presence — conversation, activities, and structured routines that reduce isolation.
Scheduled relief so family caregivers can work, rest, or attend to other responsibilities.
Essential housekeeping, laundry, and household organization that keep the home safe and manageable.
Cueing, prompting, and general supervision, including medication reminders when authorized.
Meal planning, cooking, clean-up, and hydration support aligned with the member's routine.
Shopping, appointment coordination, and accompaniment in the community when authorized and accepted.

Service Line Two
Naviah Care Services LLC is currently accepting personal care clients. Wisconsin Medicaid billing for personal care services is not yet available. Personal care is delivered based on an RN assessment, an individualized plan of care, and RN oversight.

Inquiries and referrals are welcome now. Wisconsin Medicaid billing for personal care services is not yet available, so payment is arranged privately or through another source you arrange.
Choosing Support
Many members need one or the other; some will need both. If you are not sure, send a referral or call and we will help you sort it out.

Scope of Services
Services depend on assessment, written authorization or service plan, staffing availability, worker competency, and payer or program requirements. Submitting a referral does not guarantee acceptance, authorization, or a start of care.
Wisconsin Medicaid billing for personal care services is not yet available, and we do not claim participation with any insurer or payer that is not expressly listed.
Skilled home health services are not currently offered by Naviah Care Services LLC. When a member needs clinical care, we coordinate respectfully with the member's team so the right provider is involved.
