Applications include contact information, work eligibility answers, experience, availability, employment history, references, and an optional resume. We do not collect Social Security numbers or banking information through this website.

Referrals include referrer contact details and information about the member's location, funding source, requested services, and care needs, plus any documents you attach.

How we use information

We use submitted information to evaluate caregiver applications, respond to referrals, coordinate intake, and communicate with you about your submission.

By submitting a form you consent to being contacted by phone, text, or email about that submission.

Section 03

How information is stored and protected

Submissions are transmitted over an encrypted connection and stored in a secured database that is not readable by website visitors.

Uploaded resumes and referral documents are stored in private file storage. They are not publicly accessible and cannot be listed or downloaded from the public website.

Access is limited to Naviah Care Services personnel who need the information to do their work.

Sharing

We do not sell information submitted through this website.

We share information only as needed to deliver or coordinate services, with service providers that support our operations, or where required by law.

Retention

We retain applications and referrals for as long as needed for hiring, service coordination, and applicable recordkeeping obligations, then dispose of them securely.

Your choices

You may ask us to correct or delete information you submitted through this website by contacting our office. You may also ask us to stop contacting you.

Section 07

Health information

Information you share about a member's care needs is treated as confidential. Please submit only the information necessary for us to evaluate a referral, and confirm you are authorized to share it before you do.
